La educación sexual es aún un tabú en nuestra sociedad, es algo difícil de realizar, principalmente por relacionarse a una fase muy delicada de la vida, como es la adolescencia.
La educación sexual es compleja, pues no se limita a percibir la maduración física para la relación sexual que llega con la pubertad, sino que involucra factores psicoafectivos, la prevención de enfermedades transmitidas sexualmente, la prevención del embarazo precoz y del abuso sexual.
Saber que el cuerpo está preparado físicamente para la relación sexual es un hecho, pero ¿cómo podemos saber si nuestro hijo está preparado psicoafectivamente? ¿Hasta dónde podemos interferir?
La orientación sexual va a depender del desarrollo intelectual del adolescente y de lo que la familia percibe en relación con su interés sexual. Debe hacerse sin prisa y por partes, evitando un volumen grande de información. Con la televisión, las revistas, las películas y las canciones transmitidas actualmente, la curiosidad de los niños despierta cada vez más tempranamente. Para todos los padres, sus hijos son siempre muchachos y muchos evitan inclusive imaginarse a sus hijos manteniendo relaciones sexuales, lo cual es perfectamente normal. Cuando se trata de muchachos con síndrome de Down, es comprensible que se duplique ese sentimiento, lo que hace que la educación sexual trabajada en casa sea más difícil y más importante que para los demás muchachos.
El primer paso para la educación sexual es reconocer y aceptar que el adolescente está preparado físicamente para una relación sexual. Resulta fundamental aceptar esto para asumir la necesidad de educar al adolescente. El segundo paso es la apertura al diálogo. El joven necesita saber que puede hacer preguntas y que le serán respondidas de manera clara.
Vamos a detenernos aquí y referirnos específicamente a la persona con síndrome de Down. Aún no existe una «fórmula» ideal para la educación sexual del adolescente con síndrome de Down. Sin rodeos, se trata de un terreno pantanoso y lleno de controversia, y aún en mi caso, siendo médico en rol de educadora, siento la angustia de los padres en relación con las dudas: ¿Mi hijo tendrá suficiente independencia para mantener una relación estable? ¿Debo privarlo de la relación afectiva por ese motivo?
Entonces, hablando específicamente de la persona con s. de Down, el segundo paso, el de la apertura del diálogo, debe darse en el momento en que el adolescente formule preguntas, revele señales de interés sexual o esté sufriendo algún tipo de acoso. El consenso parece ser que los padres no deben adelantarse a esas señales, evitando estimular al adolescente con s. de Down a iniciar las relaciones sexuales, y sí orientándolo paulatinamente, pues la represión del sentimiento tiende a producir comportamientos sexuales inadecuados – como la masturbación excesiva – y la agresividad.
A la par con la apertura del diálogo necesitamos:
1. Construir la identidad sexual de hombre y de mujer y reforzar el manejo de algún tipo de defensa o rechazo ante personas extrañas con actitudes sospechosas. Esto para evitar los temibles abusos sexuales – homo o heterosexuales – dirigidos a personas con s. de Down.
2. Un ambiente con menos estímulos sexuales – esto dependerá de la postura de la familia.
3. Revisar las actividades recreativas y como el adolescente está utilizando su tiempo libre: ¿está pasando mucho tiempo delante del televisor?
Los adolescentes con s. de Down que viven en entornos sin tantos estímulos tienden a construir relaciones de amistad y a no interesarse tanto por relaciones sexuales propiamente dichas. El comportamiento sexual habitual de la persona con s. de Down es de pasividad. En la gran mayoría de los adolescentes, el interés por el sexo opuesto es pueril, lo que justifica la educación sexual dirigida a grupos en particular, cuando haya realmente una justificación – tal como hemos citado anteriormente.
Las investigaciones apuntan a que los varones adolescentes son usualmente infértiles, habiendo una serie de factores asociados, como oligoospermia o azoospermia (reducción o ausencia de espermatozoides), muchos relacionados con hipogonadismo (genitales pequeños) y diversas alteraciones hormonales.
En el caso de las adolescentes, habitualmente poseen maduración y funcionamiento sexual sin grandes altera-ciones, habiendo registros de frecuencia aumentada en los ciclos anovulatorios. Teóricamente, una mujer portadora de s. de Down tiene 50% de probabilidad de tener un bebé con s. de Down. Considerando que muchos fetos con s. de Down son abortados espontáneamente, se esperaría que más del 50% de los nacidos vivos no tengan s. de Down.
Por lo tanto, la base de la educación sexual del adolescente con s. de Down consiste en la formación de la identidad hombre-mujer, la imposición de los límites -como al trabajar sobre una masturbación excesiva generada la mayoría de las veces por el ocio-, y la prevención del abuso sexual. La prevención de las enfermedades transmitidas sexualmente y los métodos anticonceptivos son un segundo plano, destinado a quienes desarrollan una vida sexual activa.
