En el Hospital Infanto Juvenil Carolina Tobar García, un equipo liderado por el licenciado Daniel Orlievsky puso a punto una técnica que, desafiando ideas establecidas, está arrojando resultados alentadores y atrae la atención de especialistas tanto en el país como en el exterior.
En 2005, el grupo recibió el Premio de la Facultad de Psicología de la UBA «Aportes de la psicología a la problemática de la niñez» y hoy interesa a referentes como Rosalind Picard, del Massachussetts Institute of Technology, y Elizabeth Torres, neurocientífica de la Universidad de Rutgers, que acaba de publicar en la revista Frontiers of Neuroscienceun método para detectar el autismo precozmente.
«Estoy muy impresionada por los esfuerzos de Daniel Orlievsky para ayudar a chicos [con autismo] para sobreponerse a los enormes desafíos de comunicación -respondió Picard ante una consulta vía mail de LA NACION-. Creo que muchos podrían beneficiarse de este enfoque.»
«Quedamos todos muy impresionados con el progreso de estos niños -coincidió Torres, también vía mail-. Realmente, el licenciado Orlievsky le encontró «la llave» a cada niño y abrió su potencial. Es un trabajo excelente, que nos gustaría poder extender acá […] Su equipo tiene una dedicación, una paciencia y un amor increíbles. Pero lo que lograron va más allá de todo eso.»
La historia de esta aventura empezó cuando Orlievsky viajó a Estados Unidos para formarse en una terapia desarrollada en Australia a fines de los años setenta por Rosemary Crossley y que en ese momento no estaba disponible en la Argentina.
«Crossley trabajaba con «soporte físico», es decir que les enseñaba a escribir a chicos con parálisis cerebral tomándoles la mano -cuenta el especialista-. Pero cuando llegué a la Universidad de Syracuse, me enteré de que la técnica era muy controvertida, porque si alguien es tomado de la mano no se sabe quién escribe, si el médico o el paciente.»
Tras la frustración, sin embargo, Orlievsky decidió hacer un intento.
Regresó al país y, para evitar cualquier tipo de duda y preservar su nombre, buscó una institución pública para montar un programa piloto a la vista de otros profesionales. Fue así como llegó al Tobar García.
«Sin darnos cuenta, fuimos reinventando la técnica hasta lograr que los pacientes, aun necesitando apoyo al comienzo, se volvieran independientes y comenzaran a escribir solos», explica el investigador, que en el hospital trabaja en forma totalmente ad honorem .
El autismo es una patología que dificulta la comunicación y se presenta en distintos grados de gravedad. Suele aparecer durante los tres primeros años de vida y es cuatro veces más frecuente en los varones de todos los grupos étnicos, sociales y económicos.
Las personas que lo padecen pueden manifestar movimientos repetitivos, inusual apego a objetos y resistencia al cambio de rutinas. En algunos casos, muestran comportamientos agresivos o autoagresivos. Parecen sufrir retardo mental, incapacidad de aprendizaje o problemas de audición.
Entre las pistas que pueden alertar a los padres está la imposibilidad de establecer contacto visual o de requerir la atención conjunta (en los bebes, señalar algo que les llama la atención para que el papá o la mamá lo miren). A veces parece que no respondieran a la voz humana. Más tarde, tienen dificultad para entender metáforas, el doble sentido, el chiste.
«Los padres lo detectan tempranamente, pero los pediatras no les prestan atención», explica Orlievsky. Según datos internacionales, es más frecuente que el síndrome de Down.
Tampoco se conocen las causas que lo originan. Hay quienes piensan que tiene raíces netamente biológicas; otros lo atribuyen a factores ambientales; otros, a la función materna. «Entre las causas orgánicas, se mencionan la rubeola congénita, la fenilcetonuria (un desorden por el cual el organismo no metaboliza adecuadamente un aminoácido, la fenilalanina), los genes… pero no hay ninguna totalmente probada -destaca Orlievsky-, porque todavía no existe un examen que permita diagnosticarlo.»
El Programa de Rehabilitación Comunicacional intenta habilitar el lenguaje a través de la escritura.
«Los chicos que nos llegan a nosotros son los más graves -cuenta-: no hablan, no pueden ejecutar una orden sencilla, no son capaces de reconocer una figura… Generalmente, se piensa que una persona primero habla y después escribe. Nosotros hacemos al revés. Dimos por tierra con un paradigma, porque en general se cree que si no se desarrolló el lenguaje hasta los cinco años el pronóstico es funesto. Sin embargo, nosotros trabajamos con chicos mayores de esa edad. Y lo que hemos logrado lo hicimos trabajando en sesiones de apenas media hora, una vez por semana.»
Las anécdotas que desgrana Orlievsky, producto de más de una década de trabajo en el hospital, son llamativas. «Una chiquita que llegó a los ocho años pudo controlar los esfínteres a los diez, el día que logró escribir la palabra «baño» -cuenta-. A partir del tratamiento comienza a cambiar la conducta; muchos empiezan a hablar, aunque no todos logran el lenguaje oral.»
El especialista ensaya algunas hipótesis para explicar estos efectos: «Una posibilidad es que el cerebro se va organizando y permite adquirir funciones que estaban bloqueadas -propone-. Por otro lado, la escritura permite utilizar recursos visuales cuando estas personas tienen problemas de procesamiento auditivo. Una de nuestras teorías es que los chicos están viviendo una sobrecarga emocional tan fuerte que no pueden relacionarse con el otro. Nosotros les ofrecemos un trabajo predecible, que les permite organizarse y en la gran mayoría de los casos dejan de ser agresivos.»
